Heritage Grove

A Partnership of Yakima Valley Memorial Hospital and Central Washington Comprehensive Mental Health

Heritage Grove is a 97-bed skilled nursing facility, providing the highest level of comfort-driven, quality care to those with complex and long-term medical and psychiatric needs. The facility offers programs designed to serve populations whose needs cannot be met in a traditional nursing home setting. While younger residents are welcome in specific situations, most residents are over age 50.

Providing a homelike environment, patients and their families depend on the health care team at Heritage Grove for assistance with adult daily living skills with conditions like Alzheimer’s disease, along with rehabilitation, medication management, nutritional counseling and behavioral intervention.

Health MattersAs a licensed skilled nursing facility, Heritage Grove provides all skilled nursing care including medication management, ostomy management (a surgically-created opening for the removal of waste), G-tube feeding, IV therapies, wound management and rehabilitation therapies and assistance with daily living skills. Also available are in-house psychiatric care and individualized behavior management planning that is integrated into the comprehensive treatment program.

At Heritage Grove, each resident is recognized as a unique individual who wants to make choices about how they live. Requiring physical assistance does not mean they forfeit the right to make those decisions. Using a person-directed approach to care, the health care staff stresses flexibility and creativity in developing each resident’s specific care plan.

Heritage Grove’s staff works directly with Senior Adult Services, Rehabilitation Programs and Acute Care Services at Central Washington Comprehensive Mental Health and staff at Yakima Valley Memorial Hospital Psychiatric Services. Joint staffing and collaborative programming among all groups ensures smooth transfer of residents from one level of care to another.

Memorial’s pharmacy provides daily delivery of medications and on-site pharmacist visits for Heritage Grove. A pharmacist attends interdisciplinary team meetings and works directly with physicians and nurses to assure safe, appropriate medication treatment for residents.

Heritage Grove social workers and nurses have well-developed relationships with DSHS Home and Community Services staff and many of the adult family home providers in the county. Heritage Grove also provides consultation and assistance to other nursing facilities and residential care programs throughout Central and Southeastern Washington.
